I'd lean towards the killer, I don't see why anyone running for their life would stop to mark up multiple men on the exact same spot. If you're sprinting away from a maniac through a mess of sleeping people, do you make a point of stepping only on their forearms on your way?
I would think for a killer, it would be to make a suspicious mark look less suspicious. For a victim... it could be perhaps not a sprint, but rather, a mark of where they were.
If the mark was left on people that were perhaps - either uninjured, or perhaps people who they thought may be less likely to be acting upon any urges, it could also be to mark a path. There were a lot of us in the flower garden... but so far, only two of us that I have spoken to with the mark.
